Secondary forms include infections, vasculitis, drug reactions, and malignancies. Key diseases reviewed include: - <strong>Acute Eosinophilic Pneumonia (AEP):</strong> Acute febrile illness with hypoxemia, diffuse opacities, BAL eosinophilia >25%, often affecting young adults; rapid corticosteroid response; rarely peripheral eosinophilia early. - <strong>Chronic Eosinophilic Pneumonia (CEP):</strong> Subacute to chronic symptoms; peripheral eosinophilia common; chest imaging shows peripheral opacities ("photographic negative" of edema); relapsing course; corticosteroids effective. - <strong>Allergic Bronchopulmonary Aspergillosis (ABPA):</strong> Hypersensitivity to Aspergillus fumigatus in asthmatics or cystic fibrosis; diagnosed by elevated IgE, Aspergillus-specific IgE, eosinophilia, and radiographic signs (bronchiectasis, mucoid impaction); treated with corticosteroids ± antifungals. - <strong>Eosinophilic Granulomatosis with Polyangiitis (EGPA):</strong> Rare vasculitis with asthma, peripheral eosinophilia, and multi-organ involvement; ANCA positive in some; treatment includes corticosteroids, immunosuppressants, and biologics (mepolizumab). - <strong>Hypereosinophilic Syndrome:</strong> Systemic eosinophilia with end-organ damage; treatment varies from corticosteroids to chemotherapy and targeted biologics. Drug-induced eosinophilic pneumonia, notably with daptomycin, is highlighted, requiring drug cessation and corticosteroids. Parasitic infections such as strongyloidiasis and tropical pulmonary eosinophilia are also important etiologies. Diagnosis relies on clinical presentation, lab eosinophilia (blood/BAL), imaging, exclusion of secondary causes, and sometimes biopsy. Most conditions respond well to corticosteroids but differ in prognosis and relapse risk. Understanding distinctions among these disorders is crucial for accurate diagnosis and treatment.
