Cystic Fibrosis (CF) is a rare genetic disorder affecting approximately 30,000 people in the U.S. and 100,000 globally, predominantly in North America, Europe, and Australia. Life expectancy has significantly improved, rising from 33 years in 2001 to 53 years in 2021. CF results from mutations in the CFTR gene, located on chromosome 7, which encodes a transmembrane ion channel responsible for chloride and bicarbonate ion transport. Over 2000 mutations have been identified, with F508del being the most common. The CFTR protein dysfunction leads to multisystemic complications affecting the lungs, upper airways, pancreas, liver, GI tract, endocrine system, and reproductive tract.<br /><br />Diagnosis of CF often involves newborn screening through immunoreactive trypsinogen measurement and genetic testing. Sweat testing is also commonly used. CFTR-related diagnoses include CFTR-Related Metabolic Syndrome (CRMS) and other CFTR-related disorders for non-diagnostic criteria carriers.<br /><br />Chronic management includes pulmonary therapies like bronchodilators, hypertonic saline, dornase alfa, and inhaled corticosteroids along with airway clearance techniques and long-term antibiotics for infections. CFTR modulators, such as ivacaftor, lumacaftor/ivacaftor, tezacaftor/ivacaftor, and triple therapy elexacaftor/tezacaftor/ivacaftor, have notably improved patient outcomes, reducing lung transplantation needs and increasing life quality.<br /><br />Pulmonary complications include exacerbations, hemoptysis, pneumothorax, Aspergillus infections, allergic bronchopulmonary aspergillosis (ABPA), and advanced lung disease, requiring multidisciplinary management. Chronic Pseudomonas aeruginosa infection is a critical concern due to its associated mortality and morbidity, with eradication strategies involving inhaled antibiotics like tobramycin.<br /><br />Advanced lung disease in CF is marked by diminished FEV1 and severe respiratory conditions, necessitating lung transplant evaluation and ongoing comprehensive care.<br /><br />In summary, CF stems from CFTR gene mutations with significant clinical variability, and advancements in CFTR modulator therapies have markedly enhanced patient prognosis and life quality.
