Pdf Summary
The Bronchoscopy Workshop, led by Dr. Saadia A. Faiz in June 2025, provides a comprehensive overview of bronchoscopy, including its indications, contraindications, techniques, and associated complications. Bronchoscopy is primarily indicated for persistent cough, hemoptysis, localized wheezing, unexplained hoarseness, suspected tracheo-esophageal fistulas, chest trauma, and diaphragmatic paralysis. It serves as an adjunct for airway management and helps localize bronchopleural fistulas, evaluate lung transplant recipients for rejection, deliver brachytherapy, and conduct research.

Contraindications include refractory hypoxemia, recent myocardial ischemia, poorly controlled heart failure, hemodynamic instability, exacerbated asthma or COPD, and cardiac arrhythmias. Special considerations are required for patients on anticoagulation therapy or with conditions like thrombocytopenia and renal insufficiency.

The workshop distinguished between flexible and rigid bronchoscopes. Flexible bronchoscopes, requiring only 10% of the trachea's cross-sectional area, allow ventilation in awake patients and can be used with local or moderate sedation. Rigid bronchoscopes offer greater suction but require an endotracheal tube and specialized expertise.

Complications during bronchoscopy may include a drop in PaO2, especially in mechanically ventilated patients, and rare occurrences of death due to hemorrhage, cardiovascular events, or other causes. Procedures associated with bronchoscopy, such as Bronchoalveolar Lavage (BAL) and various biopsies, are used to diagnose infections, malignancies, and other lung diseases.

The session also covered airway stents for tracheobronchial obstructions caused by malignant or benign conditions. Different interventions are available for these obstructions, with potential complications including stent migration and infection. The workshop highlighted the importance of understanding the complexities involved in bronchoscopy to manage pulmonary complications effectively, particularly in oncologic patients.
