This is airway anatomy using a high-fidelity simulator. Using the simulator, we will again demonstrate a systematic airway inspection, identifying the segments of the right and left tracheobronchial tree, beginning with our right-sided anatomy. We will drive through the trachea to the proximal right main stem. Turning our scope clockwise and flexing, we can see the right upper lobe orifice, the anterior, the apical, and the posterior segments. We withdraw our scope slightly, return to a neutral position, and advance into the bronchus intermedius. Turning counterclockwise and flexing into the right middle lobe. Here we can see our lateral and our medial segments. As we withdraw our scope back to the bronchus intermedius, across from the right middle lobe takeoff, we see our superior segment of our right lower lobe. We withdraw our scope again, advancing to the right lower lobe basilar segments with our medial segment, our anterior segment, our lateral segment, and our posterior basal segment. We reverse these movements, coming back to the trachea, driving down our left main stem, flexing to the left upper lobe, to our apical posterior segment, our anterior segment, withdrawing slightly to our lingular bronchus, our superior segment, our inferior segment. As we withdraw our scope, we will again see our superior segment across from our lingula, as seen here. As we advance into our basilar segments, we have a combined anterior medial, a lesion in the airway, leading to the lateral and the posterior segments of the left lower lobe. We will withdraw our scope to the trachea, avoiding any trauma to our endobronchial lesion.
